To define between whether a Post Incident Debrief form or an Ops Assurance form is to be filled out please note the following:
  • If as an officer you were directly involved in the incident then you are required to fill out a Post Incident Debrief form.
  • If as an officer you were there to Ops assure/observe/mentor then you are required to fill out an Ops Assurance form.
